For PTAs transferring a current PTA license from another state
(If you appear to be eligible for grandfathering or transferred a license from another state – you may choose to apply for a PTA license via either route.)
• Go to https://fortress.wa.gov/doh/hpqa1/hps3/Physical_Therapy/forms.htm
• Choose the link to Physical Therapist or Physical Therapist Assistant License Application Packet
• Print it out
• Read the first two sections: “General Instructions Checklist” and “Physical Therapist or Physical Therapist Assistant License Requirements”
By July 1, 2008:
Fill out and submit the five-page “Application for Physical Therapist or Physical Therapist Assistant,” along with the $100 application fee and a photograph (follow photograph instructions/guidelines on that page – page 5 – of the application packet)
Also by July 1, 2008:

As soon as possible (but may be done after July 1, 2008 and not necessarily in this order):
• HIV/AIDS Training – If you haven’t completed the required seven hours of HIV/AIDS training, find a course and, after you’ve completed it, send a copy of your completion certificate to the Department of Health. Visit http://www.doh.wa.gov/cfh/HIV_AIDS/Prev_Edu/license-training.htm to find a course that meets the requirement.
• Contact the school where you received your PTA degree and request that they send a copy of your official transcript to the Dept. of Health.
• "Physical Therapy Interim Permit Checklist" and "Sponsor Form" – These don’t apply to you.
• “FSBPT Score Transfer Service” information and “Score Transfer Request” – No matter where you took the test in the past, the Federation of State Boards of Physical Therapy in Virginia now administers all PTA licensure exams and maintains past scores from test sites around the country. So the FSBPT is where you will need to make the request to get your score transferred. You may do this online or by mail.
• “Employment Verification” – Have each employer for whom you worked in the last two years fill out one of these forms and have them send the forms directly to the Department of Health.
• “Physical Therapy License Verification” – You need to have all current and previous health care practitioner license information sent to the Department of Health. Contact every state where you have ever held a health care practitioner license and get them to send verification of the license (even if it’s an expired license) to the Board of Physical Therapy. You may ask the other states to use the form in the licensure packet or have them use their own. Some states charge a fee for this service.
• “Physical Therapist Assistant (PTA) License Waiver of Examination” – This form doesn’t apply to you.
